You can use this document to create a Living Will. A Living Will is used to advise your friends, family and medical professionals caring for you of your wishes regarding your healthcare if you become seriously ill and lose mental capacity. The document contains both advance statements and advance directives. An advance statement let’s you say what type of care you want if you are not able to make decisions or communicate. You can say where you would like to be cared for, the type of food you prefer or if you have religious or spiritual beliefs that should be observed in your care. An advance decision cannot be used to request illegal acts, such as ending your life through euthanasia, or demand care considered inappropriate by your healthcare team. This document is suitable for use in England, Wales and Northern Ireland only.
